There are many tools available that address some or all of what you have listed. For a start you might look through the "Tools" category on RQNG. This recent posting by Michael Larson might be useful: www.requirementsnetwork.com/node/1577. To this list I would also add RequisitePro, Enterprise Architect, Optimal Trace (recently acquired from Micro Focus by Compuware so this link might cease to function in the future), CaliberRM, and many other tools too numerous to mention.
